Report a possible side effect.

At Westpath we take the safety of our patients and consumers seriously. We know that side effects can happen but it is not always possible to predict who will experience them. For this reason, it is vital for us to continuously monitor the safety of our products.

To report a side effect email safety@westpath.com or call 555-515-515.

Please include the following when you report the adverse event:

1. Initials of the person experiencing the possible side effects and/or other identifying information like date of birth.

2. Person reporting the possible side effect.

3. A description of the possible side effect such as symptoms experienced and date of the possible side effect.

4. Name of the Westpath product involved.
